I consent to the collection of the data required by this application form, my photograph and, where applicable, my fingerprints, all of which are mandatory for the examination of the application and may be supplied to and processed by the competent Italian authorities for a decision on my visa application.

I understand this data, and any decision on my application, may be stored in the information system of the diplomatic and consular offices of the Ministry of Foreign Affairs and made accessible to the competent national and Schengen authorities for visa, immigration and asylum purposes, and, under defined conditions, to designated authorities and Europol for the prevention and investigation of serious offences.

I am informed of my right to be notified of the data registered about me and to request that inaccurate data be corrected or unlawfully processed data be deleted; the national supervisory authority is the Guarantor for the Protection of Personal Data.

I declare that all the information I have provided is complete and accurate. I am aware that any false statement may lead to the rejection of my application or the annulment of a visa already granted, and to legal action under Italian law. I understand that the grant of a visa does not in itself guarantee entry, and gives no right to compensation if entry is refused for failing to meet the conditions in force.
